Formation and Licensing

The process of formation or redomestication and licensing of a captive insurer in Oklahoma is designed to be relatively straight forward.

  1. Contact Steve Kinion, Director of Captive Insurance Division, at 918-704-8136 or Steve.Kinion.CTR@oid.ok.gov to arrange a meeting with the Captive Insurance Division staff to discuss your proposed captive.
  2. Prepare and file documents necessary for the organization of your entity. The services of a local lawyer are not necessary but may be desired.
  3. Obtain all necessary forms directly from our website: Forms
  4. Submit one copy of all forms and exhibits to the Oklahoma Insurance Department, Captive Insurance Division at oidcaptive@oid.ok.gov for review. Include a $200.00 review fee and a $300.00 license fee.
